Home Care Service

Our home care service has commenced since 1995. It is a unique direct community care service conducted by a group of professional nurses for the people living with HIV/AIDS (PLWHA) and their carers. According to the need and readiness of the clients, the service could be provided at the patients’ homes, hospitals and AIDS clinics. The scopes of the service include medication supervision, education on safer sex and risk reduction, rehabilitation, emotional support, psychosocial support and bereavement counseling for carers. Treatment goal and individual case plan will be mapped out in accordance with the different needs and specific requirement of each client aiming at improvement of their mental and health status.

In addition, education and support are also provided to the carers of the PLWHA. This unique service assists the carers to alleviate their anxieties regarding HIV/AIDS and helps them to know more about how to take care of the patients at home. Through the intensive contacts, our home care nurses fill in the gap where hospitals and clinics cannot cater for needs of PLWHA outside clinic hours. Nevertheless, we provide a 24-hour emergency service for those who need urgent consultation and assistance after office hours. This 24 hour service is especially important for the newly diagnosed and those who have started new regimen of HAART (highly active anti-retroviral therapy).
